Job summary

Rockport Healthcare Services seeks a Speech-Language Pathologist to provide direct patient care in a skilled nursing facility rehab setting. You will evaluate, treat, and document patient progress, collaborate with families and physicians, and contribute to interdisciplinary care.

The role also involves supervising clinical staff, ensuring regulatory compliance, and enhancing patient outcomes through evidence-based practices.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited SLP program.
• California SLP license or license candidate.
• 2-3 years experience in a skilled nursing facility rehab environment.
• Excellent written and verbal communication skills.

Responsibilities

• Provides treatment programs to maximize speech, language, cognitive, and swallowing abilities.
• Evaluates and treats patients and documents services in medical records.
• Communicates with families, physicians and team members; participates in rehab planning.
• Supervises Clinical Fellow and other SLPs in direct patient care.
• Writes clear and complete documentation including goals and progress.
• Collaborates with interdisciplinary team and contributes to rehab conferences.
