The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist (SLP) to deliver and oversee speech therapy services, including assessment, program development, and in-service education under physician orders.

Qualifications

• Master's degree in Speech-Language Pathology is required.

Responsibilities

• Provides and directs Speech Therapy services to patients, including assessment and treatment.
• Conducts screenings to determine need for intervention or treatment.
• Develops and implements treatment plans with short and long-term goals.
• Communicates evaluation findings to patients, family, and team members.
• Coordinates treatment plans with patient, family, and other team members.
• Monitors patient response and adjusts interventions per physician orders.
• Assists with discharge planning and home/community programming when applicable.
• Documents assessment, treatment, follow-up, and termination of services.
