The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist to provide and direct Speech Therapy services to patients, including assessment and treatment, program development, and related professional in-services, under physician orders.

The SLP will perform screenings, develop treatment plans with short/long-term goals, document outcomes, participate in discharge planning, and supervise students as required. A Master’s degree and state licensure are required.

Qualifications

• Master’s Degree in Speech-Language Pathology.
• Current state licensure or license eligible.
• Demonstrates good skills in all forms of communication.

Responsabilités

• Provides Speech Therapy services to patients and initiates referrals.
• Conducts screenings of patients at regular intervals to determine need for intervention/treatment.
• Evaluates patients to obtain data necessary for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and other team members.
• Develops treatment plans by identifying a problem list with short and long-term goals.
• Coordinates treatment plan with patient, family, and other team members.
• Monitors patient’s response to intervention and modifies treatment as indicated.
• Recommends and obtains equipment to improve patient’s communication; instructs on use of augmentative devices.
• Participates in discharge planning.
• Develops home/community programming to maintain performance.
• Documents results of assessment, treatment, and termination of services.
• Supervises students and volunteers per state regulations.
• Maintains ongoing continuing education and professional development.
• Records indirect/billable treatment times and submits statistics to regional office.
• Provides treatment per standards of practice and infection control.
