The Grand Rehabilitation and Nursing at Queens is seeking a licensed Speech Language Pathologist (SLP) to provide and direct Speech Therapy services for patients. This role includes assessment, treatment planning, program development, and collaboration with physicians and the care team.

Responsibilities include screenings, evaluations, designing goals, coordinating plans with families, and documenting progress.

Formación

• Masters degree in Speech-Language Pathology required.
• Current state licensure or licensure eligible.
• Demonstrates good skills in all forms of communication.
• Maintains education and development appropriate for position.

Responsabilidades

• Provides Speech Therapy services to patients and initiates referrals when appropriate.
• Conduct screenings of patients to determine need for intervention/treatment.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and other team members.
• Establishes patient schedules in conjunction with team members.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plans with patient, family and team members.
• Monitors patient progress and modifies treatment as needed.
• Recommends equipment and trains patient, family and staff in use of communication devices.
• Participates in discharge planning.
• Develops home or community programming to maintain progress.
• Documents results and maintains records.
• Supervises students and volunteers as required.
• Maintains ongoing continuing education and professional development.
• Records indirect and billable treatment times and reports to regional office.
• Follows infection control and safety standards.
