The Grand Rehabilitation and Nursing at Queens is actively seeking a Speech Language Pathologist (SLP) to provide and direct speech therapy services, including assessment and treatment, under physician orders.

The role involves conducting screenings, developing treatment plans, coordinating care with patients and families, documenting outcomes, and supervising students/volunteers as required. Compensation: $43-$48 per hour.

Qualificações

• Master's degree in Speech-Language Pathology required.
• Current state licensure or licensure eligible.
• Demonstrates strong communication abilities with patients and families.
• Commitment to ongoing education and professional development.

Responsabilidades

• Provides Speech Therapy services to patients and initiates referrals as appropriate.
• Conducts screenings to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets evaluation findings to patients, family, and team members.
• Establishes patient schedules in collaboration with the team.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment with patient, family and team members.
• Monitors progress and modifies treatment to achieve goals per physician orders.
• Assists with equipment and instruction for augmentative communication devices.
• Participates in discharge planning and home/community programming as needed.
• Documents assessment, treatment, follow-up and termination of services.
