Qualifications

• Master's degree in Speech-Language Pathology from ASHA-accredited program.
• CFY completed under supervision of a licensed SLP.
• Hawaii SLP license required at hire.
• Clinical Competence in Speech-Language Pathology Certificate required at hire OR CCC-A for Audiology.
• Basic Life Support certification required at hire.
• National Provider Identifier (NPI) required at hire.

Responsibilities

• Provide evaluation, diagnosis and care plans for cognitive, communication, and swallowing disorders.
• Deliver direct or indirect therapy in hospital, SNF, clinic, or home care and coordinate referrals.
• Participate in case management and discharge planning.
• Educate professional staff and develop group treatment materials for clients and caregivers.
• Maintain reports with outcomes for caseload management and QA.
