Job summary

Yukio Okutsu State Veterans Home in East Hawaiʻi seeks a Speech-Language Pathologist to improve residents’ communication, cognition, and swallowing safety. Qualifications

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Hawaii SLP license in good standing.
  • CCC-SLP from ASHA.
  • CFY completion and current BLS certification.
  • Preferred experience in skilled nursing, geriatric rehabilitation, and Medicare documentation.
  • Strong clinical judgment, communication, documentation, and interdisciplinary collaboration skills.

Responsibilities

  • Conduct comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing disorders.
  • Develop individualized treatment plans and deliver evidence-based interventions.
  • Evaluate and manage dysphagia with diet modifications and therapeutic exercises.
  • Educate residents, families, and caregivers on plans and progress.
  • Maintain accurate documentation and compliance with regulatory standards.
  • Collaborate with physicians and care teams on discharge planning and outcomes.

Join Yukio Okutsu State Veterans Home and be part of a team that proudly cares for our friends, family, and neighbors across East Hawaiʻi.

The Speech-Language Pathologist (SLP) enhances residents' quality of life by improving communication, cognitive function, and swallowing safety through evidence-based evaluation and treatment. Working collaboratively with the interdisciplinary care team, this role promotes functional independence, safe nutrition and hydration, and successful discharge planning while ensuring compliance with clinical, regulatory, and reimbursement standards.

Key Responsibilities
  • Conduct comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing disorders, and develop individualized treatment plans.
  • Deliver evidence-based speech-language pathology interventions and monitor resident progress, modifying care plans as needed.
  • Evaluate and manage dysphagia, including recommendations for diet modifications, swallowing strategies, and therapeutic exercises.
  • Educate residents, families, and caregivers on treatment plans, progress, and strategies to support communication and swallowing function.
  • Maintain accurate, timely, and compliant clinical documentation while meeting productivity expectations and regulatory requirements.
  • Collaborate with physicians and the interdisciplinary team on resident care, discharge planning, and quality outcomes while adhering to infection prevention and patient safety standards.
Education, Experience & Certifications
  • Master's degree from an accredited Speech-Language Pathology program.
  • Current Hawaii Speech-Language Pathologist license in good standing.
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
  • Completion of a Clinical Fellowship Year (CFY) and current Basic Life Support (BLS) certification.
  • Preferred experience in skilled nursing, long-term care, geriatric rehabilitation, and Medicare documentation requirements.
  • Strong clinical judgment, communication, documentation, and interdisciplinary collaboration skills with a commitment to ongoing professional development.
