Ohana Pacific Health in Hawaii seeks a part-time Speech Language Pathologist (3 days per week) to develop and implement a progressive home health rehabilitation program. Qualifications

• Master's degree in Speech/Language Pathology from ASHA-accredited program.
• Current Hawaii license to practice as a Speech Language Pathologist.
• Current Basic Life Support CPR certification (AHA).
• 2-step TB clearance required.
• Home health experience preferred.

Responsibilities

• Evaluate and admit patients; determine impairment and prognosis; plan and implement treatment; reassess and discharge when goals are met.
• Develop Home Health Aide care plans; supervise and delegate to HHAs; manage caseload and referrals.
• Provide direct patient care and education; involve patients and families in treatment plans.
