Job summary

PACS in Sunnyvale is seeking a licensed Speech Language Pathologist (SLP) with CCC to join an in-house therapy program. Opportunities include full-time, part-time, or PRN roles.

The SLP will develop and implement treatment plans, treat patients per physician directives, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. The role requires strong documentation, coordination of care, and clinical preceptorship for CFY/RFP students.

Qualifications

• SLP CCC only; CFY not eligible.
• Bachelor’s degree in Speech-Language Pathology required; advanced degrees preferred.
• Proficient with computer data entry, e.g., Point Click Care.

Responsibilities

• Develop and obtain approval for treatment plans from referring physicians.
• Treat patients according to physician plans.
• Communicate patient progress with supervisor and health team members.
• Participate in conferences and staff training as needed.
• Document evaluations, treatment notes, and progress per policy and state practice act.
• Instruct families and nursing staff on maintenance and discharge preparation.
• Participate in discharge planning and supervise CFY/RFP SLPs and students.

Education

Bachelor’s degree in Speech-Language Pathology

Master’s or Doctorate in Speech-Language Pathology preferred

Job Description:

• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
• Participate in in-services training program for other staff in the facility.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist in the Board State Practice Act and governmental and third-party payer requirements.
• Record treatment changes per policy and procedures.
• Instruct patient’s families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Participate in discharge planning.
• Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologist and student programs.
• Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Required license or certification:

• Bachelor’s degree in Speech-Language Pathologist Therapy, prefer graduates of Masters or Doctorate Program in Speech-Language Therapy. Proficient in computer skills such as inputting data into programs such as Point Click Care.
