Job summary

Sunnyvale Gardens Post Acute is seeking a Speech Language Pathologist (SLP) to join our in-house therapy program in Sunnyvale, CA. We welcome full-time, part-time, or PRN arrangements for qualified providers.

Applicants must have SLP CCC and no CF; a strong clinical background and ability to work collaboratively with the care team are essential. We offer competitive pay and comprehensive benefits in a supportive work environment.

Qualifications

• Bachelor's degree in Speech-Language Pathologist Therapy; Masters/Doctorate preferred.
• SLP CCC only; CF candidates not considered.
• Proficient with Point Click Care software.

Responsibilities

• Develop treatment plans and obtain physician approval.
• Treat patients per the physician plan and update progress.
• Communicate with supervisor and health team about patient progress.
• Participate in care conferences and rehab meetings as needed.
• Support staff training and in-service programs in the facility.
• Document evaluations and treatment notes per policy and law.
• Educate families and staff for discharge planning.
• Provide clinical precepting for CFY/RFP candidates.

Education

Job Description

• Develop effective treatment plan and obtain approval for services from referring physician.

• Treat patients per the physician treatment plan.

• Communicate with supervisor and other health team members regarding patient progress, problem and plans.

• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.

• Participate in in-services training program for other staff in the facility.

• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist in the Board State Practice Act and governmental and third-party payer requirements.

• Record treatment changes per policy and procedures.

• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.

• Participate in discharge planning.

• Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologist and student programs.

• Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures.

• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Required license or certification

• Bachelor's degree in Speech-Language Pathologist Therapy, prefer graduates of Masters or Doctorate Program in Speech-Language Therapy. Proficient in computer skills such as inputting data into programs such as Point Click Care.
