A Home Health Speech Language Pathologist provides personalized speech, voice, language, hearing, and swallowing therapy in patients’ homes or facilities. They evaluate, develop, and review treatment plans, administer diagnostic tests, and monitor patient progress. The role includes instructing patients and families on communication strategies, maintaining documentation, and participating in care coordination and discharge planning.
