Job summary

Bayhealth, Inc. is seeking a Per Diem Home Health Speech Language Pathologist. The SLP provides evaluation and treatment to patients at their homes, develops and coordinates the plan of care, and teaches patient, family, and caregivers preventative and rehabilitative procedures.

The role includes conducting evaluations, creating care plans, administering therapy, and collaborating with physicians and the care team to ensure comprehensive, coordinated care in a home health setting.

Qualifications

• Requires master’s or doctoral degree in SLP.
• State license or licensure eligibility as an SLP.
• Minimum one year of clinical SLP experience.
• CPR certification current.
• Valid driver's license with insured vehicle.

Responsibilities

• Conduct evaluations and assess home environment; identify equipment needs for speech/language function.
• Identify functional speech defects and establish a plan of care to improve the patient’s function.
• Administer the speech therapy program using techniques and equipment.
• Recommend communication devices as indicated.
• Consult with referring physicians and care team to ensure coordinated, comprehensive care.
• Participate in case conferences and other required meetings to ensure coordinated care.
• Perform other duties to facilitate delivery of speech-language pathology services.

Bayhealth, Inc. is seeking a Per Diem Home Health Speech Language Pathologist. The Speech Language Pathologist provides evaluation and treatment to patients at their homes. The Therapist develops and coordinates the plan of care and teaches patient/family and caregivers preventative and rehabilitative procedures.

Responsibilities

• Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
• Identifies functional speech defects and establishes a plan of care to improve the patient's function.
• Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
• Recommends communication devices/aids as indicated.
• Confers with a referring physician (or other authorized licensed independent practitioner) and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
• As appropriate, participates in case conferences, inservice and other meetings required to ensure coordinated and comprehensive care.

Qualifications

Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:

• Is licensed as a speech language pathologist by the State in which the individual furnishes such service; or
• If the State does not license speech-language pathologists:
• Must successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);
• Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and
• Successfully completed an approved national examination in SLP
• A minimum of one year's clinical experience in Speech–Language Pathology.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
