Job summary

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to deliver in-home therapy, assess and treat communications and swallowing disorders, and collaborate on treatment plans. Experience with home health and documentation is essential.

You will work under the Clinical Manager, evaluating patients at home, coordinating care, and contributing to discharge planning while maintaining high professional standards and patient confidentiality.

Qualifications

• Licensed Speech-Language Pathologist with current credentials.
• Experience delivering in-home therapy.
• Ability to develop and review plans of treatment.
• Strong communication with patients and families.
• Ability to maintain documentation and reports.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on use of alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required.
• Participate in care coordination and discharge planning.
• Maintain confidentiality of patient information.

As a Home Health Speech Language Pathologist, You Will

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor and/or the physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy.
