Job summary

LiveWell Homecare Agency, LLC is seeking a dedicated Speech-Language Pathologist to deliver personalized home health services across Florida. You will assess, diagnose, and treat communication and swallowing disorders, develop care plans, and educate patients and families to support functional improvement.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA), minimum six months of experience, valid license, CPR certification, reliable transportation, and a valid driver’s license.

Responsibilities

• Evaluate, develop, and review treatment plans for patients.
• Assess and treat speech, voice, language, hearing, and swallowing disorders.
• Administer diagnostic tests and monitor patient progress.
• Instruct patients on communication aids; provide counseling and education to patients, families and healthcare staff.
• Maintain accurate documentation and participate in care coordination and discharge planning.
• Ensure confidentiality of patient information.
