Job summary

Qualifications

• Master’s or doctoral degree in SLP.
• Licensed as an SLP in the state where services are provided, or meet state criteria including a national exam.
• Proficient computer skills with Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours of referral or 1 business day; document reasons for any delay.
• Develop and obtain approval for treatment plans from the referring physician.
• Treat patients per the physician plan and document progress.
• Supervise CFY/SLP assistants in direct care as allowed by state law.
• Communicate patient progress with supervisor and health team members.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide in-services to facility staff on therapy processes.
• Record evaluations, daily treatment and progress notes per policy.
• Instruct families and nursing staff on home programs and discharge planning.
• Act as a clinical preceptor for CFY/RFP students when applicable.

Education

Master’s or doctoral degree in Speech-Language Pathology

Licensed as an SLP by the state or eligible for national exam
