Job summary

HCA Florida Putnam Hospital in Palatka, FL seeks a Speech Language Pathologist to provide outpatient SLP services for pediatrics and adults, including acute care bedside swallow evaluations and FEES training support.

Qualifications

• Licensed Speech-Language Pathologist in Florida.
• CCC-SLP certification required or eligible.
• Master's degree in a related field.
• Basic Life Support must be obtained within 30 days of employment start date.

Responsibilities

• Evaluate communication, cognitive, and swallowing abilities using structured assessments and observations.
• Develop and adjust individualized treatment plans to improve communication and safe swallowing.
• Document evaluations, daily notes, progress updates, and discharge summaries accurately and timely.
• Educate patients and families on therapy goals, safety recommendations, home strategies, and expected progress.
• Collaborate with physicians, nursing, rehabilitation staff, and care managers to support coordinated care.
