Job summary

Interim HealthCare Inc. is seeking a licensed Speech-Language Pathologist to diagnose and treat communication and swallowing disorders in patients served.

The role involves collaborating with physicians and the interdisciplinary team to develop and implement individualized plans of care, assess progress, and adjust treatments as needed. The position requires a Master’s or Doctoral degree in Speech-Language Pathology, state licensure (or eligibility), and two years of relevant experience within

Qualifications

  • Master’s or doctoral degree in speech-language pathology and licensure as a Speech-Language Pathologist or eligibility in a state that licenses SLPs.
  • Two years of experience as an SLP in the last five years.
  • Valid driver’s license and auto liability insurance if driving is required.
  • IHE/PHI access and background checks as required by policy and law.

Responsibilities

  • Assist physician or licensed practitioner in evaluating communication, hearing and swallowing function and developing the care plan.
  • Complete initial and ongoing comprehensive assessments.
  • Teach task-oriented therapeutic activities across modalities (oral, signed, written).
  • Assess changes and modify services; obtain revision orders as needed.
  • Document treatment progress in required formats per standards and payor requirements.
  • Educate patient and family to promote progress and discharge planning.
  • Collaborate with interdisciplinary team to coordinate care and discharge planning.
