Job summary

Silverwood Post Acute in Phoenix, AZ seeks a licensed Speech Language Pathologist to evaluate residents within 24 hours of referral, develop treatment plans, and coordinate care with physicians and family teams.

You will supervise Speech Language Pathologist Assistants, document progress, participate in discharge planning, and ensure compliance with state practice acts and facility policies.

Qualifications

• Bachelor's degree in Speech Language Pathologist Therapy. Licensure as a Speech Therapist in the state.
• Master’s or Doctorate preferred.
• Ability to read technical procedures and manuals.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop effective treatment plans and obtain physician approval.
• Supervise Speech Language Pathologist Assistants in patient care.
• Document evaluations and daily progress notes.
• Discharge planning and caregiver training for discharge.
