Job summary

Personal Healthcare Management LLC in New York seeks a licensed Speech-Language Pathologist to provide skilled rehabilitation therapy in a long-term care setting. You will evaluate residents, develop and implement plans of care, and collaborate with physicians and the interdisciplinary team.

The role requires NYS licensure, ASHA certification or CFY, and at least 1 year of SNF experience; familiarity with Medicare/Medicaid and clinical software is preferred.

Qualifications

• Graduate of an accredited school of Speech-Language Pathology with a Master’s Degree or higher.
• Valid ASHA certification in Speech-Language Pathology or participating in Clinical Fellow Year Program.
• Current unencumbered NYS license as SLP or CFY with supervision.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Speech-Language Pathology evaluations upon physician order.
• Establishes SLP plan of care for treatments per physician order.
• Documents progress toward SLP goals and completes required reports.
• Complies with Medicare Part A PPS case management and regulatory requirements.
