Job summary

Personal Healthcare Management LLC is seeking a Speech-Language Pathologist to provide services for speech, voice, swallowing and language disorders in a skilled nursing setting. The role involves evaluations, plan of care development, and ongoing treatment per physician orders, with documentation to meet regulatory requirements.

Candidates must hold a NYS SLP license (or CFY with supervision) and ASHA certification (or CFY).

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current NYS licensure as a Speech-Language Pathologist or Clinical Fellow Year with supervision.
• ASHA certification or participation in CFY program.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Speech-Language Pathology evaluations per physician order.
• Establishes and manages plan of care for SLP treatments.
• Documents progress toward goals and completes required re-certifications.
• Ensures compliance with Medicare/Medicaid and facility policies.
