Job summary

Personal Healthcare Management LLC seeks a Speech-Language Pathologist for a long-term care setting in New York. You will screen residents, perform SLP evaluations, and establish care plans per physician orders.

You will deliver treatments, document progress, and adjust programs based on resident response. The role requires NYS licensure, ASHA certification or CFY participation, and at least 1 year in long-term care.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Valid NYS license as a Speech-Language Pathologist or CFY supervision.
• ASHA certification or CFY participation.
• Minimum 1 year of experience in a long-term care facility.
• CFY supervision under a certified SLP may be required.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete Speech-Language Pathology evaluations per physician order.
• Establishes Speech-Language Pathology plan of care for treatments per physician order.
• Provides Speech-Language Pathology treatments and modalities.
• Modifies treatment program level, intensity and techniques based on resident response.
• Documents resident progress toward Speech-Language Pathology goals, including discharges and certifications.
• Demonstrates knowledge of Medicare Part A PPS case management.
• Complies with federal and state regulations related to rehabilitation therapy.
• Utilizes clinical software applications per policies and procedures.
• Completes care plans and MDS as directed, per facility policy.
• Assists in ordering equipment and participates in discharge planning with the interdisciplinary team.
• Submits reports within established time frames.

Education

Master's degree in Speech-Language Pathology

NYS Licensure as Speech-Language Pathologist

ASHA Certification

Clinical Fellow Year (CFY) participation
