Job summary

Excellent Home Care Inc. in the New York City area seeks a dedicated Speech Language Pathologist to provide in-home rehabilitation services to adults and pediatric patients. You will collaborate with a multidisciplinary team to assess needs, develop plans of care, document progress, and coordinate visits with nursing staff.

NY State licensure and ASHA certification are required. This role offers a compassionate work environment and opportunity to make meaningful differences in patients' daily

Qualifications

• Master’s degree in Speech Language Pathology.
• At least 1 year of SLP experience in health care.
• Active New York State SLP license and ASHA certification.

Responsibilities

• Provides Speech Language Rehabilitation services to patients in their homes.
• Works with the health team to set goals and plan activities.
• Maintains patient records and documents treatments and progress.
• Reports initial findings, goals, and visit frequency within 24 hours of visits.
• Communicates progress with nurses and coordinates care.
• Re-evaluates patients as needed and completes progress reports for discharge.
