Creative Solutions in Healthcare, Inc. is seeking a licensed Speech-Language Pathologist to join our in-house team in Texas.

This full-time role focuses on resident-centered care, evaluating swallowing and cognitive needs, and delivering individualized therapy plans to improve independence and communication. Responsibilities include AAC implementation, caregiver education, collaboration with care staff, and accurate documentation.

Qualifications

• Active license in Speech-Language Pathology in Texas.
• Good standing with regulatory boards.
• SNF/Long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing goals.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech-language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for individuals with severe expressive or language disorders.
• Educate clients, families, and caregivers as a subject matter expert.
• Collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate competent technical skills per current Speech Therapy standards of practice.
• Communicate changes in resident condition.
• Maintain accurate billing records and clinical documentation.
• Respond to family requests and concerns in a timely manner.
• Coordinate and provide training with Restorative Nursing and other staff.
• Assure resident safety and confidentiality of information.
