Oakwood Care and Rehabilitation in Lakewood, CO, is a post-acute facility providing long-term care, short-term care, and geriatric services. We are seeking a dedicated Full Time Speech-Language Pathologist for our in-house rehab program to join a stable, patient-centered team.

Qualifications

• Speech-Language Pathologist license required.
• Open to SLPs at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care, developing care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
