Job summary

Heritage Gardens Rehabilitation and Healthcare in Carrollton, TX, is seeking a dedicated Speech-Language Pathologist for our in-house rehab program. This full-time role involves screening, evaluating, and delivering therapeutic interventions to patients with physical functioning disorders, while adhering to regulatory and clinical standards.

You will collaborate with care teams, prepare discharge summaries, and support families and caregivers.

Qualifications

• Speech-Language Pathologist license is required.
• Open to SLPs at all experience levels. New Grads are welcome!

Responsibilities

• Screening and evaluation of patients with physical functioning disorders and/or wound care, developing care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Regular standing and walking throughout the facility with occasional lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of the rehabilitation plan (manual techniques, ambulation, exercises, modalities, assistive devices, wound care).
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Generate discharge summaries of services provided, ensuring regulatory compliance.
