Acts Retirement-Life Communities is seeking a Speech Language Pathologist for Spring House Estates in Lower Gwynedd, PA. You will evaluate residents, develop treatment plans, provide therapy, and discharge planning within a chuyên care continuum.

Qualifications

• Master’s degree in Speech Language Pathology required.
• Active CCC-SLP from ASHA.
• Valid SLP license or eligibility in applicable state(s).
• Experience in skilled nursing or outpatient rehab preferred.

Responsibilities

• Provide speech therapy services to residents including evaluations, planning individual treatment programs and goals, actual treatments, and discharge planning.
• Collaborate with care teams to coordinate care across inpatient, home health, outpatient, and wellness services.
• Engage in on-site training and CEUs to support professional development.
