Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist (SLP) to provide and direct speech therapy services to patients, including assessment, treatment planning, and program implementation. The SLP will work under physician orders and coordinate care with the health care team.

Responsibilities include screenings, evaluations, developing treatment plans, educating patients and families, and discharge planning.

Qualifications

• Master's Degree in Speech-Language Pathology.
• Current state licensure or license eligible.

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals as appropriate.
• Conducts screenings of patients to determine need for intervention or treatment.
• Evaluates patients to obtain data for treatment planning.
• Interprets evaluation findings to patients and families.
• Develops treatment plans with short and long-term goals.
• Coordinates treatment plan with patient, family and team members.
• Monitors patient progress and adjusts treatment as needed.
• Instructs patients and families in use of communication devices.
• Participates in discharge planning and home/community programming.
• Documents assessment, treatment, follow-up, and termination of services.
• Supervises students and volunteers as required.
• Records indirect and billable treatment times and submits statistics.
