Job summary

Apple Rehab Farmington Valley is seeking a dedicated Speech Language Pathologist to join our team. This full-time role located in Plainville, CT involves providing high-quality resident care and enhancing communication and swallowing abilities for our residents.

You will evaluate and treat residents, develop care plans, and educate families and staff, while adhering to regulatory standards and PPS guidelines. Prior long-term care experience is preferred and professional growth is encouraged.

Qualifications

• Graduate of accredited SLP program and eligible for licensure.
• CCC certification or CFY status required/in progress.
• Prior long-term care experience preferred.

Responsibilities

• Evaluate, develop, organize and implement resident care programs with speech therapy.
• Manage caseload and productivity, schedule treatment for individuals or groups.
• Instruct residents, families and caregivers for post-discharge care.
• Ensure adherence to PPS guidelines and proper documentation.
• Provide supervision and education to affiliating students and support staff.
• Maintain safe, sanitary treatment environments and equipment.
