Job summary

Sierra Valley Rehab Center in Porterville, CA is seeking a dedicated Speech Language Pathologist (SLP) for a part-time role to improve residents' communication and swallowing abilities in a skilled nursing setting.

The ideal candidate has a Master’s degree in SLP, California license, CCC-SLP preferred, and experience in post-acute care. You will join an interdisciplinary team and provide direct therapy, documentation, and caregiver training.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and valid California SLP license.
• CCC-SLP certification preferred.
• Prior experience in skilled nursing or post-acute settings preferred.
• CPR certification is required.

Responsibilities

• Evaluate patients for speech, language, cognitive-communication, and swallowing disorders.
• Develop and implement individualized treatment plans in coordination with the care team.
• Provide direct therapy and caregiver/staff training as needed.
• Maintain accurate documentation including daily treatment notes, evaluations, progress reports, and discharge summaries.
• Participate in care planning meetings and collaborate with nursing and therapy staff.
• Ensure compliance with all applicable regulations and facility standards.
