Job summary

The Grove at Valhalla Rehabilitation and Nursing Center in New York is seeking a Speech Language Pathologist (SLP) to assess, diagnose, and treat communication disorders in residents under physician orders.

You will conduct screenings, develop and implement individualized treatment plans, supervise students, and participate in discharge planning while maintaining thorough documentation and compliance with standards.

Qualifications

• Masters Degree in Speech-Language Pathology

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals when appropriate.
• Conducts screenings of patients at regular intervals to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients and family and team.
• Establishes patient schedules with input from the team.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family and team members.
• Monitors response to intervention and modifies treatment as indicated.
• Recommends and obtains equipment to improve communication and instructs use of AAC devices.
• Participates in discharge planning.
• Develops home/community programming to maintain function.
• Documents results of assessment, treatment, follow-up, and termination.
• Supervises students and volunteers as required.
• Continues ongoing education and professional development.
• Records indirect and billable treatment times per procedures.
