Job summary

Responsibilities

• Provides and directs Speech Therapy services to patients including assessment and treatment.
• Conducts screenings of patients at regular intervals to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and team members.
• Develops treatment plans with short and long term goals and methods to achieve them.
• Coordinates treatment plan with patient, family and team members.
• Monitors patient progress and adjusts treatment to achieve goals per physician orders.
• Recommends equipment and trains patient, family and staff in use of augmentation devices.
• Participates in discharge planning and develops home/community programming.
