• Interprets and communicates evaluation findings to patients, families, and team members.
• Establishes individual patient schedules with the team.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family and team.
• Monitors progress and adjusts treatment per physician orders.
• Recommends equipment and trains on augmentative communication devices.
• Participates in discharge planning and home/community programming.
• Documents results of assessments, treatment, follow-up and termination of services.
• Supervises students and volunteers as appropriate.
• Maintains confidentiality and adheres to standards.
