Job summary

The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services to patients, including assessment and treatment under physician orders.

The SLP will screen and evaluate patients, develop and implement treatment plans, coordinate with families and the care team, monitor progress, and document results while maintaining infection control standards.

Qualifications

• Masters Degree in Speech-Language Pathology.
• Current state license or license eligible.
• Demonstrates good skills in all forms of communication.

Responsibilities

• Provides and directs Speech Therapy services to patients including assessment and treatment.
• Conducts screenings to determine need for intervention.
• Evaluates patients to obtain data for treatment planning.
• Interprets and communicates evaluation findings to patients and team.
• Establishes individual patient schedules in conjunction with other team members.
• Develops treatment plans and coordinates with patient and family.
• Monitors patient response and adjusts treatment per physician orders.
• Recommends and obtains equipment to improve patient’s communication.
• Participates in discharge planning.
• Develops home or community programming to maintain patient progress.
• Documents results of assessment, treatment, follow-up, and termination of services.
• Supervises students and volunteers in accordance with state regulations.
• Assumes responsibility for ongoing continuing education and professional development.
• Records indirect and billable treatment times and submits statistics.
• Provides treatment per standards of practice and infection control requirements.
• Assists with Quality Assurance Programs as required.
• Adheres to confidentiality standards.
