Job summary

TEEMA Solutions Group is seeking a Speech-Language Pathologist (SLP) to evaluate, diagnose, and treat patients across diverse clinical settings. You will develop individualized plans to improve communication, cognition, and swallowing safety while promoting independence.

Requires a Master’s in Speech-Language Pathology, active state licensure, and CCC‑SLP preferred or required by facility policy. Experience in SNF, rehab, hospital, or school settings is advantageous, with emphasis on

Qualifications

  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current SLP license in the state of practice.
  • CCC‑SLP from ASHA preferred or required according to facility policy.
  • Current Basic Life Support (BLS) certification preferred or required by facility policy.

Responsibilities

  • Evaluate patients with speech, language, cognitive, communication, voice, and swallowing disorders.
  • Develop and implement individualized treatment plans based on patient needs and established goals.
  • Provide evidence‑based speech‑language pathology interventions to improve communication, cognition, swallowing function, and overall quality of life.
  • Perform speech, language, cognitive, voice, and swallowing assessments utilizing standardized evaluation tools and clinical judgment.
  • Conduct modified swallowing evaluations and other swallowing assessments when within scope of practice and facility requirements.
  • Monitor patient progress and modify treatment plans based on clinical response and rehabilitation goals.
  • Educate patients, families, and caregivers regarding treatment plans, home exercise programs, communication strategies, and swallowing precautions.
  • Maintain accurate, timely, and complete clinical documentation in accordance with facility policies, regulatory standards, and professional practice guidelines.
  • Collaborate with physicians, physical therapists, occupational therapists, nursing staff, case managers, educators, and other healthcare professionals to coordinate patient care.
  • Participate in interdisciplinary care planning, discharge planning, quality improvement activities, and continuing education.
  • Maintain patient safety and comply with HIPAA, infection prevention, and facility policies.
  • May provide therapy services in various settings based on facility needs.
  • Perform other job-related duties as assigned.
