Job summary

Aurora Health Care in Wisconsin is seeking a licensed Speech-Language Pathologist to evaluate and treat patients with speech, language, voice, cognitive and swallowing disorders. You will develop written plans of care and monitor outcomes to ensure effective therapy.

You will discharge planning, provide family coaching and coordinate referrals while documenting evaluations, progress notes and billing per policy.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Wisconsin SLP license (or eligibility) prior to hire.
• Basic Life Support (BLS) certification for Healthcare Providers.

Responsibilities

• Interpret physician referrals and evaluate patients with speech, language, voice, cognitive and swallowing disorders.
• Develop and implement written care plans and monitor outcomes.
• Discharge planning, family coaching and community referrals as indicated.
• Document evaluations, progress notes and billing per policy.
• Maintain safety, infection control and participate in improvement activities.
• Possibly supervise non-licensed personnel following compliance requirements.
• Collaborate with physicians and care team to optimize patient care.
• Meet productivity standards and billing/documentation timelines.
