• Document evaluations, sessions, progress notes and discharge summaries per regulations.
• Educate patients and families on treatment plans and safety strategies.

Job description

Seeking a Part Time Speech Language Pathologist (SLP) to join the in-house rehab team at Santa Maria Post Acute,a specialized 55-bed Skilled Nursing Facility in Santa Maria, CA!Santa Maria Post Acute in Santa Maria, CA offers short-term rehab and long-term care. Ideally needing 25 hours/week consistently. All availability welcomed!

SLP Responsibilities:

• Evaluate, diagnose, and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders
• Develop and implement individualized treatment plans based on patient needs and physician orders
• Provide skilled therapy services to improve communication, cognition, and safe swallowing function
• Perform clinical swallow evaluations and recommend appropriate diet modifications and compensatory strategies
• Collaborate with interdisciplinary team members (PT, OT, nursing, physicians) to ensure coordinated care
• Document evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards
• Educate patients, families, and caregivers on treatment plans, safety strategies, and carryover techniques
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
• Ensure compliance with all federal, state, and facility guidelines (e.g., Medicare, Medicaid, HIPAA)
• Participate in care planning meetings and contribute to discharge planning
• Maintain productivity standards and meet documentation timelines

SLP Requirements:

• Masters degree- Speech Language Pathology
• Valid SLP and CCC-In good standing with all regulatory agencies and licensing boards
