Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Speech Language Pathologist to work in patients' homes in California. You will evaluate patients, develop care plans, deliver therapy, and coordinate with the care team to improve communication and quality of life.

The role is per diem with a rate of $150 per visit, with flexible scheduling and potential to convert to full-time based on performance. Mileage reimbursement and paid leave benefits are offered.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.
• Valid driver’s license and insured, reliable vehicle.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
