Job summary

Caringhand Home Health Solutions, Inc is seeking a Speech-Language Pathologist to work in patients' homes, delivering individualized speech therapy and coordinating with physicians and care teams. Travel to patient homes is required; field-based, per diem position with compensation of $150 per visit.

We value teamwork and patient-centered care, with a flexible schedule and opportunities for conversion to full-time based on performance. California licensure and CPR are prerequisites.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.
• Valid driver's license and insured, reliable vehicle.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $150 per visit.
