Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Speech Language Pathologist to work in patients’ homes, delivering individualized speech therapy and collaborating with a clinical team to improve communication and quality of life.

The role requires a California SLP license, at least 1 year of clinical experience, and CPR certification. The position is per diem with a $150 per visit rate and offers flexible scheduling and potential to convert to full-time based on performance.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license and CPR certification (AHA).
• Minimum 1 year of clinical SLP experience.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
