Job summary

Premier Home Health Providers Inc. is seeking a Speech-Language Pathologist to deliver in-home therapy across California. You will evaluate patients, create personalized care plans, and collaborate with physicians to ensure coordinated, patient-centered care in a home setting.

The role offers a per-diem schedule at $150 per visit, with potential to convert to full-time based on performance. A flexible schedule, mileage reimbursement, and opportunities for professional development are provided.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.
• Valid driver's license and insured, reliable vehicle.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
