Job summary

H2 Health Prime Living is seeking a PRN Speech-Language Pathologist in Yardley, PA to deliver 1:1 patient care for a geriatric population. The role focuses on evaluating and treating dysphagia and cognitive/neurologic conditions with an independent and team-based approach.

Qualifications

• Education: Graduate of an accredited Speech Language Pathology program.
• Active CCC-SLP state license required.
• Prior outpatient therapy experience a plus; dysphagia experience required.

Responsibilities

• Provide 1:1 therapy to a geriatric caseload in outpatient settings.
• Evaluate and treat patients with dysphagia, cognitive impairment, and neurologic conditions.
• Collaborate with interdisciplinary team to plan and adjust care.

PRN (4-10 visits/week) - Yardley, PA

This position will evaluate and treat a primarily geriatric caseload 1:1 with a mix of dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient's functional needs.
