Job summary

H2 Health seeks a Speech-Language Pathologist to join our Hillsville outpatient team in Virginia. You will evaluate, treat, and support patients with communication, cognitive, and swallowing needs, collaborating with a multidisciplinary team to maximize function and independence.

Qualifications

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current Virginia Speech-Language Pathologist license or eligibility for licensure.
  • Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to complete Clinical Fellowship Year (CFY).
  • CPR certification or willingness to obtain as required.

Responsibilities

  • Perform comprehensive speech-language evaluations and develop individualized plans of care.
  • Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders.
  • Provide evidence-based interventions designed to improve communication skills, cognitive function, and overall quality of life.
  • Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients and families on treatment strategies, home programs, and techniques to support continued progress.
  • Maintain accurate and timely documentation of patient progress and treatment outcomes.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.

H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Hillsville, Virginia.
