Job summary

H2 Health Prime Living - 1:1 Patient Care in Senior Living Communities is a PRN position offering 4-10 visits per week. You will evaluate and treat a predominantly geriatric caseload with dysphagia and cognitive needs, working both independently and within teams to provide personalized care.

H2 Health supports professional growth with clinical mentorship, in-house education through H2 University, and a flexible schedule with no weekends or holidays, along with a full benefits package.

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Experience in outpatient therapy is a plus. Dysphagia experience is required.
• Active CCC-SLP state license.

Responsibilities

• Provide 1:1 patient care for a geriatric caseload.
• Deliver therapy for dysphagia, cognitive impairment, and neurologic disorders.
• Work independently and within interdisciplinary teams to tailor plans.
• Make decisions based on each patient’s functional needs.
