Job summary

WarsawRehabilitation And Healthcare Center in Warsaw, NC is hiring a Speech Therapist to provide comprehensive evaluations and therapy under physician orders. The role emphasizes documentation, treatment planning, and collaboration with the interdisciplinary team.

CFY completion and state licensure, ASHA recognition, and dedication to patient confidentiality are required. You will participate in meetings, mentor students, and ensure compliant care while delivering compassionate therapy.

Qualifications

• CFY completion required.
• Licensed or eligible for state licensure in North Carolina (ASHA recognized).
• ASHA certification or eligibility recommended.

Responsibilities

• Provide comprehensive speech therapy evaluations per physician orders.
• Document findings on standardized evaluation formats with accuracy.
• Develop treatment plans with long- and short-term goals, frequency, duration and modalities.
• Obtain physician orders for evaluations, treatments, recertifications and discharges.
• Document progress and update treatment plans monthly; document discharges and in-services as indicated.
• Attend facility meetings and participate in interdisciplinary teams; supervise CFYs and students as required.
• Maintain patient confidentiality and HIPAA compliance; ensure timely documentation.

Qualifications

1. 1.Graduate of an accredited university with an MA, or M.S. in communication disorders that is
2. recognized by ASHA and state board.
3. 2.Holds current license and/or registration in the state (as applicable).
4. 3.Successful completion of CFY.

Job Responsibilities:

1. 1.Provide comprehensive speech therapy evaluations as per MD orders.
2. 2.Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
3. 3.Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
4. 4.Ensure MD orders are obtained for evaluations, treatments, recertification and discharges.
5. 5.Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
6. 6.Document patient’s progress in approved format accurately and timely.
7. 7.Document monthly updated treatment plan in standardized format in timely manner.
8. 8.Document patient’s discharge and in-service as indicated.
9. 9.Document any caregiver or discharge in-services in approved format as indicated.
10. 10.Complete any and all other documentation required by Facility.
11. 11.Document care plans and information needed for the MDS as deemed necessary by the facility.
12. 12.Attend facility and rehab meetings as indicated by supervisor and/or Regional Manager.
13. 13.Participate in facility committee meetings as indicated.
14. 14.Act as a rehab delegate to the interdisciplinary team.
15. 15.Participates in scheduling of patients.
16. 16.Perform screens as indicated.
17. 17.Complete billing timely and bill patients ethically and accurately for all speech therapy services rendered.
18. 18.Cover other therapists’ patient caseload during absence as necessary.
19. 19.Supervise junior staff members or students in compliance with State ST Board and Federal regulations and requirements. Co-sign notes of CFY’s and students as indicated as per state and federal guidelines.
20. 20.Maintain professional relationship with co-workers, facility staff, patients and patient families.
21. 21.Provide in-service education regarding patient care or speech therapy service areas to rehab staff, facility staff and community as needed.
22. 22.Comply with all facility policies and procedures and safety guidelines.
23. 23.Comply with patient confidentiality and Federal Resident Rights and HIPAA legislation.
24. 24.Report to work on time, adhere to scheduled hours and project a professional image at all times.
25. 25.Adhere to productivity requirements, complete productivity reports and submit in a timely manner.
26. 26.Comply with all Facility Human Resource policies i.e. lunch, overtime, sick and time off notification.
27. 27.Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers.
28. 28.Perform any other duties assigned by the department supervisor, lead therapist or Regional Manager.
29. 29.Adhere to any and all other Facility written and oral systems and operations.
