St. Joseph Rehabilitation & Care in Norfolk, NE is seeking a Full-Time Speech Language Pathologist for our in-house rehab program.

The role involves screening, evaluating, and delivering rehabilitative speech-language therapy to geriatric patients, with emphasis on regulatory compliance and quality care. You will develop and adjust care plans, communicate progress to patients and families, and collaborate with the rehabilitation team to ensure comprehensive discharge planning.

Qualifications

• Active licensure as a Speech-Language Pathologist.
• Open to candidates at all experience levels.

Responsibilities

• Conduct screening and evaluation of patients with communication disorders per clinical standards.
• Deliver rehabilitative speech-language interventions aligned with regulations and best practices.
• Develop and implement care plans including documentation and discharge summaries.
• Provide counseling to patients and families regarding therapy options and progress.
• Collaborate with care teams to coordinate patient-centered rehabilitation.

St. Joseph Rehabilitation & Care is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Speech Language Pathologist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
