St. Joseph Rehabilitation & Care in Norfolk, NE is hiring a Full Time Physical Therapist to join our in-house rehab program.

The role focuses on screening, evaluating, and treating patients with physical functioning disorders, and coordinating care plans to support recovery. Responsibilities include delivering skilled physical therapy, employing manual techniques, therapeutic exercises, wound care as applicable, and generating discharge summaries in compliance with regulatory standards.

Qualifications

• Licensed Physical Therapist.
• This position is open to PTs at all experience levels.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and wound care, developing care plans meeting regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients in line with regulatory and clinical practice requirements.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other providers regarding physical disorders.
• Generate discharge summaries of services, ensuring regulatory and clinical compliance.

St. Joseph Rehabilitation & Care is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
