Job summary

Excellent Home Care NY is seeking a Speech Language Pathologist to provide in-home rehabilitation for patients in the New York City area. The role involves planning, implementing, and coordinating speech therapy services with patients, families, and healthcare teams to improve communication and swallowing function.

The SLP will evaluate, treat, and re-evaluate patients, maintain detailed documentation, and collaborate with nursing and other professionals to adjust plans as needed.

Qualifications

  • Master’s degree in Speech Language Pathology.
  • At least 1 year SLP experience in health care; home care desirable.
  • Current NY licensure as SLP and ASHA CCC.

Responsibilities

  • Plan and provide speech and language rehabilitation in patients' homes.
  • Collaborate with the health team to set goals and activities.
  • Maintain patient records and discharge summaries.
  • Communicate progress with the nurse and update care plans.
  • Perform assessments and re-evaluations as needed.

Speech Langauge Pathologist (SLP) Job Responsibilties:
  • Planning and provision of Speech and Language Rehabilitation services to Patients in their homes; and for working with the Patient, family and/or others to provide training and/r assistive devices to eliminate or reduce areas of deficit, usually associated with verbalization of thoughts and ideas and swallowing difficulties.
  • Work with other professional staff to establish rehabilitation and maintenance goals and activities.
Speech Langauge Pathologist (SLP) Job Duties:
  • Evaluates the Speech and Language Rehabilitation needs of the Patients and provides Speech and Language Rehabilitative Services for Patients as ordered by the Patient’s Authorized Medical Practitioner.
  • Performs clinical tests to assess Speech and Language disorders and potential for restoration. Develops a Plan of Care with restorative and/or maintenance goals to meet identified needs.
  • Instructs Patients/Family and other appropriate persons/health team personnel in the use of assistive modalities and appropriate Speech and Language Procedures.
  • Maintains records for each Patient treated by documenting general information, special precautions, description of and number of treatments rendered, reaction to treatment, progress or lack of progress made and Discharge Summary.
  • Works with health team to shorten the Patient’s recovery from illness and the management of residual disability.
  • Makes telephone contact, followed by a written report with the Nurse to report initial findings, goals, visit frequency and estimated length of service within 24 hours of initial visit.
  • Charts all visit contacts and findings on designated forms, within twenty-four (24) hours, and as required by agency policy.
  • Communicates with the Nurse at least every two (2) weeks to discuss the Patients progress.
  • Telephones the Agency immediately with any information that will influence or change the provision of services including any unusual situations observed during the course of service.
  • Re-evaluates Patients as needed to detect progress attained and completes a written progress report for the Patient’s record containing Patient’s progress, continued treatment plan, goals, frequency of visits and outcomes of care provided.
  • Contacts the Nurse coordinating the care to obtain Medical Orders for changes in the Plan of Care or visit frequency and to report missed visits, as necessary.
  • Submits Discharge Summaries at the completion of services that include outcome of care provided, equipment ordered and any recommendations for follow-up care.
Speech Langauge Pathologist (SLP) Minimum Education:
  • Master’s Degree in Speech Language Pathology
Speech Langauge Pathologist (SLP) Minimum Experience and Skills:
  • A minimum of one (1) year Speech Language Pathology experience in a Health Care setting.
  • Demonstration of considerable knowledge of current standards, procedures and techniques of Speech Language Pathology. Home Care experience desirable.
Speech Langauge Pathologist (SLP) License/Registration/Certification/Permit Required:
  • Currently licensed in New York State as a Speech Language Pathologist. Current certification of clinical completion in Speech from the American Speech and Hearing Association.
