Job summary

TriStar Centennial Medical Center in Nashville, TN seeks a Speech Language Pathologist to provide high‑quality, patient‑centered care and to develop and conduct individualized therapy programs for communication, cognitive, and swallowing impairments.

The role involves assessments, education to patients and families, and collaboration with rehabilitation staff to optimize patient outcomes in a comprehensive center within the TriStar Health system.

Qualifications

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Speech Language Pathologist
  • Masters Degree

Responsibilities

  • Conduct a subjective assessment of the patient’s speech language abilities during spontaneous conversation and administer objective tests to assess communication assets and liabilities.
  • Structure and implement an individualized patient program to improve communication abilities.
  • Regularly evaluate patient progress during treatment and adjust the program as needed to maximize progress.
  • Document progress in the patient’s chart, complete notes, charges, discharge notes and patient communication.
  • Consult with Program Director and Rehab Physician/Medical Director regarding pre-screen assessments and physician consultations to determine placement of potential patients.

Job Summary and Qualifications

The Speech Language Pathologist provides high‑quality, patient‑centered care by developing and conducting individualized therapy programs for patients with communication, cognitive, and/or swallowing impairments. As a member of the health care team, the Speech Language Pathologist performs comprehensive patient assessments and establishes appropriate treatment goals. The Speech Language Pathologist serves as an educator to patients, families, students and colleagues.
