Job summary

Interim HealthCare of Northern Indiana is seeking a Home Health Speech-Language Pathologist (SLP) to provide patient-centered therapy in patients' homes across South Bend, IN and surrounding areas. You’ll deliver speech, language, cognitive, and swallowing therapies, assess progress, and collaborate with a home health team to meet individualized goals.

The role requires a Master’s/Doctoral degree in Speech-Language Pathology, an active Indiana SLP license, CPR certification, and at least one

• Master's or Doctoral degree in Speech-Language Pathology required.
• Active Indiana SLP license required.
• Minimum of 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, critical thinking, and empathy.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive challenges.
• Assess patient progress and adjust goals within the plan of care.
• Conduct patient assessments and document progress against goals.
• Assist with articulation, fluency, swallowing exercises, and related therapies.
• Educate patients and families on the plan of care, safety practices, and self-care.
