Job summary

Interim HealthCare is seeking a Home Health Speech-Language Pathologist (SLP) to provide home-based therapy across patient cases in California. The role focuses on improving communication, swallowing and cognition, tailored to each patient’s needs, while working within a supportive home health team.

You will assess, document progress and adjust therapy plans to meet patient goals. This full-time home-based position emphasizes autonomy, ongoing education, and patient-centered care in a home

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology is required.
• Active SLP license in CA is required.
• 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification and any others may be required.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Work with a home health team to meet plan of care and goals.
• Observe and evaluate patient communication, swallowing and information processing.
• Conduct patient assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises, fluency and reading.
• Educate patient and family on plan of care and safety.

Responsibilities

• Provide home-based speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient’s plan of care and established goals
• Observe and evaluate a patient’s ability to communicate, swallow and process information
• Conduct patient assessments, document progress and ensure patient goals are met
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading
• Educate patient and family on plan of care, therapy goals, safety practices and self-care

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology and active SLP license in CA
• Minimum of 1 year of speech therapy experience, ideally in home healthcare
• CPR certification [and any others]
• Knowledge of state and federal home health regulations
• Good clinical judgment, critical thinking skills, strong communicator, empathy and compassion
